How to Declare a Major or Minor in Epidemiology
- Before you begin, consult UR Student or CDCS to determine the schedule for the required and elective courses you intend to take. The semester information on this page reflects typical offerings and is subject to change — it should not be taken to mean that a course is offered in every semester listed.
- Complete the Intake Form for Draft Declaration of a Epidemiology Major or Minor
- Email the program administrator for epidemiology, Amanda Moore, at amanda.johnson@rochester.edu to schedule a time to review and discuss your draft declaration plan.
- After that review, submit the Registrar's official Major/Minor Declaration form to formally declare the major or minor
Please note that the plan you submit is tentative; you may make changes. After declaring, you are encouraged to revisit your plan regularly and to meet periodically with your college advisor, the epidemiology program administrator, or the epidemiology advisor to discuss changes and progress.
Important: If your course selection changes from the list that you included in your original declaration to the registrar, you must contact the program administrator or major advisor to review the change. The program must then submit a declaration change form to the registrar to document the change.
